New Client Form
In order to make your pet's first visit to Brookfield Veterinary Hospital easier, we have provided our new client information form below. Please print and fill out the form and bring it with you to your pet's first visit.
Surgery Admission Procedures
Prior to your pet's surgery, you must fill out and sign a release form authorizing us to use anesthesia. We ask that you do not feed your pet any food after 9 p.m. the evening before surgery; however, it is okay to give your pet water.
Accurate Estimates
We will give accurate and complete estimates for all procedures that require hospitalization, including dental care and surgical care. Our goal is to give you an accurate estimate of the cost of providing care for your pet. However, during the course of treatment, additional procedures may be indicated. We will call you prior to performing any additional procedures to give you a cost estimate, so that you may decide to proceed with any additional care that may be required.
Payment Policy
Payment for all services is due at the time of your pet's discharge. We accept cash, Visa, MasterCard, Discover Card and CareCredit. We do not accept checks.
